Please note: Any fees associated with medical care ordered outside of our office (e.g., labs, x-rays, specialist consultations) are the responsibility of the patient.

Individual Membership
- One comprehensive exam and lab work per year
- Up to 10 office or tele-health visits annually
Couples Membership
- One comprehensive exam and lab work per adult (over 21)
- Up to 10 office or tele-health visits annually per person
Family Membership
- Designed for families with 2 adults and 2 children (aged 13–21) living in the same household
- Includes one comprehensive exam and lab work per adult
- Up to 10 office or tele-health visits annually per adult and 5 visits per child, including one sports physical annually
Note: The Concierge Care member is responsible for any fees associated with medical care ordered or provided outside of the provider’s office visit. This would include labs, x-rays, testing, etc.
