Please note: Any fees associated with medical care ordered outside of our office (e.g., labs, x-rays, specialist consultations) are the responsibility of the patient.

Note: The Concierge Care member is responsible for any fees associated with medical care ordered or provided outside of the provider's office visit. This would include labs, x-rays, testing, etc.
